Most private health plans — whether through an employer, the ACA marketplace, or purchased directly — are legally required to cover substance use disorder treatment at parity with other medical care, under the Mental Health Parity and Addiction Equity Act. That doesn't mean every plan covers every facility or every level of care the same way.
What to actually check
Before committing to a specific facility, verify three things directly with your insurer: whether the facility is in-network (out-of-network care can mean substantially higher out-of-pocket costs), what levels of care are covered (detox, residential, PHP, IOP, and standard outpatient are often covered differently), and whether prior authorization is required before admission.
In-network vs. out-of-network
A facility accepting "private insurance" broadly doesn't guarantee it's in-network with your specific plan. Call the number on the back of your insurance card and ask directly, rather than relying on a facility's own claim of accepting your insurer.
